US Navy Relieves Truxtun Cmdr After Caribbean Collision

The US Navy recently relieved Rear Admiral James Koffi from command of the guided-missile destroyer USS Truxtun after a severe collision with the fleet oiler USNS Supply during an underway replenishment-at-sea (ORSP) operation in the Caribbean Sea on February 11, 2026. The incident occurred while both ships were docked at Ponce Naval Station in Puerto Rico.

Rear Admiral Carlos Sardiello, commander of U.S. Naval Forces Southern Command/4th Fleet, took swift action to remove Koffi from his role, citing a loss of confidence regarding his command abilities following this serious operational mishap. The incident captured the attention of naval authorities as footage revealed a sudden and intense impact with crew members shouting “We’ve been hit.”

Two sailors were taken to local hospitals for treatment due to minor injuries sustained during the collision. Cmdr. James Koffi, who had assumed command in January 2025, has been reassigned temporarily to Naval Surface Group Middle Atlantic.

Cmdr. Taylor Auclair was appointed as Koffi’s successor and is now commanding USS Truxtun. The cause of the incident remains under investigation. Despite this setback, the destroyer continues to serve U.S. Southern Command in its operational duties within the Caribbean region.

The US Navy’s swift response underscores their commitment to maintaining operational readiness amid such critical incidents. As investigations progress, details about what led to the collision will be revealed, offering a comprehensive understanding of how such accidents can occur and lessons for future operations.
